It causes tics or involuntary spasms, some of which have been so violent they have damaged his spine &#8211; the reason he is now in a wheelchair.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">Another tic means he involuntarily calls out and repeats words \u2013 sometimes the words can be offensive. He says his tics have become \u201cmore chaotic\u201d because of the stress he is under.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">He also has a learning disability and a range of additional physical and mental health conditions, including ADHD and Obsessive Compulsive Disorder (OCD).<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">On 6 October last year, Matthew was taken to hospital with a urinary tract infection.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">He had been in supported housing, but that placement had broken down, so he was not able to return.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">Since then, Matthew has been living in a side room in the hospital. Matthew is a talented artist and his parents have brought one of his paintings with them.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">He manoeuvres his wheelchair so he can get a better look at the picture which shows three boats jumbled on a bright yellow strip of beach.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">The sea and sky are vibrant blue, but Matthew points to the grey railings and pavement that run across the bottom of the picture.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">\u201cThat represents my life,\u201d Matthew says. \u201cThe barrier preventing me doing the things I want to do.\u201d<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">His parents, who have their own health issues, describe the situation as extremely distressing.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">\u201cMatthew is now in that position whereby he&#8217;s not really seen as an individual,\u201d says his mother, Christine. \u201cHe has no meaningful life.\u201d<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">Matthew\u2019s support was being funded by NHS Surrey Heartlands Integrated Care Board (ICB) under a scheme called Continuing Healthcare (CHC). Only people with the highest needs are eligible for this free NHS social care.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">After nearly two months in hospital, he received a letter from the ICB saying it had tried 145 different homes providing care and support. They identified three which could potentially take him.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">The ICB now says the family declined a number of suitable options before reaching this more formal stage.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">Matthew\u2019s parents strongly dispute this. They also say one of the three homes listed was ideal but in the end no place was offered.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">When the ICB wrote to Matthew again on 4 December 2023, he was told there was just one home where he might be able to live.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">The family says the home was totally unsuitable for someone as vulnerable as Matthew, with drug-dealing happening openly on the street outside.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">The ICB letter said it believed the placement met \u201chis assessed needs\u201d, and should he refuse it then \u201cthis means that you decline CHC funding and make your own private arrangements\u201d.<!-- --><\/p>\n<\/div>\n<div data-component=\"text-block\" class=\"sc-18fde0d6-0 dlWCEZ\">\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">CHC funding was withdrawn on 21 December.  The ICB says it made a referral to Surrey County Council for it to provide his care.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">The council says it was made aware of Matthew\u2019s case in December but did not become involved until May.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">Matthew\u2019s disabilities &#8211; and being on benefits &#8211; mean that if his care is not funded by the NHS, then it should be provided by his local authority.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">But in the following months, neither seems to have taken on the job of finding him somewhere to move to.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">It is a complex case, but we have not been given a clear answer as to why this happened.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">The consequence for Matthew is he is stuck in hospital.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">The estimated cost of a standard NHS hospital bed in 2020\/21 <!-- --><a target=\"_blank\" href=\"https:\/\/www.theyworkforyou.com\/wrans\/?id=2023-03-14.165361.h\" class=\"sc-c9299ecf-0 bZUiKB\" rel=\"noopener\">was nearly \u00a32,500 a week.<!-- --><\/a><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">Matthew\u2019s mother, Christine, describes the current situation as \u201cnot fair on anyone\u201d.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">\u201cThe amount it must be costing to keep Matthew in a hospital bed should be being spent on somewhere that he is able to live his own life and enjoy,\u201d she says.<!-- --><\/p>\n<\/div>\n<figure>\n<div data-component=\"image-block\" class=\"sc-18fde0d6-0 jFCfG\">\n<div data-testid=\"image\" class=\"sc-814e9212-1 fcEyBx\"><img decoding=\"async\" src=\"https:\/\/www.bbc.com\/bbcx\/grey-placeholder.png\" class=\"sc-814e9212-0 cCvKR hide-when-no-script\"\/><img decoding=\"async\" sizes=\"(min-width: 1280px) 50vw, (min-width: 1008px) 66vw, 96vw\" srcset=\"https:\/\/ichef.bbci.co.uk\/news\/240\/cpsprodpb\/ef68\/live\/e1070900-44ec-11ef-b74c-bb483a802c97.jpg.webp 240w,https:\/\/ichef.bbci.co.uk\/news\/320\/cpsprodpb\/ef68\/live\/e1070900-44ec-11ef-b74c-bb483a802c97.jpg.webp 320w,https:\/\/ichef.bbci.co.uk\/news\/480\/cpsprodpb\/ef68\/live\/e1070900-44ec-11ef-b74c-bb483a802c97.jpg.webp 480w,https:\/\/ichef.bbci.co.uk\/news\/640\/cpsprodpb\/ef68\/live\/e1070900-44ec-11ef-b74c-bb483a802c97.jpg.webp 640w,https:\/\/ichef.bbci.co.uk\/news\/800\/cpsprodpb\/ef68\/live\/e1070900-44ec-11ef-b74c-bb483a802c97.jpg.webp 800w,https:\/\/ichef.bbci.co.uk\/news\/1024\/cpsprodpb\/ef68\/live\/e1070900-44ec-11ef-b74c-bb483a802c97.jpg.webp 1024w,https:\/\/ichef.bbci.co.uk\/news\/1536\/cpsprodpb\/ef68\/live\/e1070900-44ec-11ef-b74c-bb483a802c97.jpg.webp 1536w\" src=\"https:\/\/ichef.bbci.co.uk\/news\/480\/cpsprodpb\/ef68\/live\/e1070900-44ec-11ef-b74c-bb483a802c97.jpg.webp\" loading=\"lazy\" alt=\"Signpost for Epsom Hospital outside the building\" class=\"sc-814e9212-0 hIXOPW\"\/><\/div>\n<\/div>\n<p><figcaption class=\"sc-8353772e-0 cvNhQw\">Epsom General Hospital has been Matthew&#8217;s home for most of the past year<!-- --><\/figcaption><\/p>\n<\/figure>\n<div data-component=\"text-block\" class=\"sc-18fde0d6-0 dlWCEZ\">\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">In May this year, eight months after Matthew arrived at the hospital, the ICB says health staff requested a review to see if he was still eligible for NHS-funded Continuing Healthcare.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">Official guidance says the threshold for this screening process \u201chas been set deliberately low, in order to ensure that all those who require a full assessment for eligibility\u201d have that opportunity.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">It also says people should be given reasonable notice when this is to happen. Experts say that would usually mean a few days.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">Matthew\u2019s family says he was given just 10 minutes\u2019 notice that an assessment or \u201cchecklist\u201d would be carried out.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">He called his parents, who live an hour away. They couldn\u2019t get there in time to support him. He says he asked for the checklist to be delayed but was told it would go ahead without him.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">We compared the checklist against previous multi-disciplinary assessments of Matthew\u2019s health and care needs. There are significant differences.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">In the checklist, his Tourette\u2019s syndrome is mentioned in passing, and there appears to be little acknowledgement of the severity or impact of his tics.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">There is also no mention of his ADHD, OCD, or the deep brain stimulator which is used to help control his tics, and which he requires help with.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">The checklist was completed using hospital notes. The ICB says it was undertaken in line with the national framework, with health professionals involved in Matthew\u2019s care.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">But the family says the member of staff involved was new and did not know him.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">\u201cIt just made me upset. So basically, they&#8217;re not listening,\u201d says Matthew.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">His family says his health has deteriorated since he was last assessed, but the conclusion of the checklist was that he was no longer eligible for CHC.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">Claire Stone, NHS Surrey Heartland&#8217;s chief nursing officer says while it cannot comment on individual cases, it works closely with those involved to \u201cprovide support and advice and ensure a smooth transition to any new arrangements\u201d. Nearly three-quarters (74%) reported a rise in local disputes over whether councils or the NHS should fund a person\u2019s care.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">They also say care staff are increasingly being expected to take on tasks which would have once been done by health professionals.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">And with their own budgets under pressure, nearly all directors say they have little confidence they will be able to meet their legal duties to support people in the year ahead.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">Tom Brown, who speaks for Adass, says the current situation is not sustainable: \u201cI think local authorities are being made the provider of last resort for people with some of the most complex needs.\u201d <!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">He says investment in community services is needed to allow health and social care \u201cto make a profound difference in people\u2019s lives\u201d.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">Matthew\u2019s parents describe the existing health and care system for people like him as broken.<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">\u201cThe impact it has is to destroy lives,\u201d says his father, Roger. \u201cIt is destroying Matthew&#8217;s life, and it is severely impacting on ours.\u201d<!-- --><\/p>\n<p class=\"sc-eb7bd5f6-0 fYAfXe\">However, Matthew remains optimistic.
